Company History
Founded in 2011 by Tiffany T. Weems, 7TH & Madison Extensions of Empowerment began as a community-driven agency committed to supporting individuals with intellectual and developmental disabilities through person-centered coordination and housing navigation.
In 2015, the organization became one of the first Support Coordination Agencies in New Jersey to transition successfully under the Division of Developmental Disabilities’ (DDD) Fee-for-Service model, establishing a trusted reputation for quality, compliance, and member advocacy.
Over the past decade, 7TH & Madison has evolved to meet the changing needs of New Jersey’s disability community—expanding into Tenancy Support Services and Assistive Technology (AT) evaluations. Today, as a RESNA member organization and NJ FamilyCare-approved provider, 7TH & Madison integrates technology, housing, and clinical expertise to promote accessibility, communication, and independent living for individuals across the state.
